Siksika Health Services Alberta

Objective: to determine the effectiveness of using a digital therapeutic solution and Remote Patient Monitoring (RPM), to prevent and reduce occurrence of diabetic foot ulcers in Siksika Nation members living with type 2 diabetes and diabetic peripheral neuropathy.

This project mapped the current reality of diabetes care at Siksika Health Services to identify inefficiencies and gaps in care. It centered around gathering information to inform the re-creation of the diabetes program at Siksika Health Services.
