PATHWAYS Gathering 2024: 
Advancing Indigenous Health Care Across Canada

In the spring of 2024, PATHWAYS convened an inspiring and impactful event at the Native Canadian Centre of Toronto. The gathering brought together members of the Advisory Circle, pilot project leaders, and organizations from coast-to-coast-to-coast. This collaboration celebrated innovation, shared best practices, and strengthened partnerships to enhance health and wellness among Indigenous communities in Canada.

The event was headlined by a keynote address from Dr. Tyler White, a member of the Siksika First Nation and a seasoned health care leader. Dr. White has served as the Chief Executive Officer for Siksika Health Services for over 22 years, leveraging his vast experience to improve First Nations health care. His keynote highlighted the importance of collaboration, community-driven solutions, and innovative approaches to addressing health disparities.

The PATHWAYS event spotlighted the transformative efforts of several pilot projects that are driving significant health improvements in Indigenous communities. These projects demonstrate the power of local solutions to address complex health challenges, particularly in managing type 2 diabetes.

Wagmatcook First Nation
This initiative integrated multi-disciplinary health care practitioners to support diabetes patients in adopting healthier lifestyles. It also addressed food security challenges, offering tailored solutions for rural communities in Nova Scotia.

Maskwacis 
Leveraging technology, the Maskwacis project connected more patients with essential resources. This included nurse practitioners who provided timely assessments and immediate care, enhancing access to critical health services.

Municipality of Baker Lake 
Focused on youth engagement, this project cultivated youth champions to educate and mobilize the community. By raising awareness and building capacity, the initiative aims to mitigate the impact of type 2 diabetes through education and empowerment.

Métis Federation of Manitoba
In a data-driven approach, this project gathered vital clinical information from 1,200 community members. The insights will represent diverse age groups and inform future health strategies across the province.

Ebb & Flow First Nation Health Authority 
A specialized community and clinical team was formed to address diabetes management. The team provided comprehensive support, linking patients with family doctors, dietitians, pharmacists, and other specialists to ensure holistic care.

Siksika Health Services 
Utilizing digital therapeutic solutions and Remote Patient Monitoring, this initiative focused on preventing diabetic foot ulcers among Siksika Nation members living with type 2 diabetes. The innovative approach has shown promise in improving patient outcomes.

Nuu-chah-nulth Tribal Council
Addressing the unique challenges of remote communities, this project facilitated travel and procured essential A1C testing devices, including the A1CNow and DCA analyzer cassettes, to improve diabetes screening and management.

The Power of Collaboration
PATHWAYS exemplifies the strength of partnerships. As a collaboration between Boehringer Ingelheim (Canada) Ltd., Bimaadzwin, and Indigenous health policy leaders, the program provides funding to advance health and wellness projects that address type 2 diabetes and chronic obstructive pulmonary disease (COPD) in Indigenous communities.

This gathering reaffirmed the commitment of PATHWAYS to empowering Indigenous Peoples with the resources, support, and innovative solutions needed to overcome health challenges. By fostering collaboration and amplifying community voices, PATHWAYS continues to pave the way for a healthier future.
